Accessing Your Health Information
At Mazqil Medical Centre, we believe every patient has the right to access their personal health information. In line with the Personal Health Information Protection Act, you may request your medical records by completing our access form. The form can be filled out on your computer, but it must be printed and signed before submission. For security reasons, electronic copies of completed forms cannot be saved, so we encourage you to print a copy for your records.
Once completed, the form can be mailed, delivered in person, or faxed to our Health Records Department. Please note that faxing is a less secure method of transmission than mailing or personal delivery. We process all requests as quickly as possible, though by law, we have up to 30 days to provide a response.
Requests for proof of birth/death/admission
If you need proof of birth, death, or admission for personal or legal purposes, Mazqil can guide you through the process.
Birth Certificate
After a birth, Mazqil provides a Birth certificate. This record is required to apply for an official birth certificate through Nigeria Population Comission. The hospital does not issue that directly. To obtain one, please visit your local government area to obtained the NPC birth certificate for your newborn.
Death Certificate
Mazqil issues a Medical Certificate of Death, which is sent with the deceased to the funeral home. The funeral home then prepares a Statement of Death and registers it with the Office of the Registrar General. Official death certificates can be obtained through Service Nigeria.
Corrections to your record
IMazqil is committed to ensuring that your health records are accurate and complete. If you believe that an error exists in your medical record, you may submit a Request for Correction to Personal Health Record form to our Health Records Department. We will review your request carefully and respond within 30 days.
